一句话理解

适应负荷,关注的是反复或长期适应过程可能伴随的生理成本。1993 年,McEwen 与 Stellar 将这个概念带入对压力和疾病关系的讨论。[1]

它提醒我们:理解身体的状态,既要看当下的反应,也要看过去一段时间里,身体如何持续应对。

短期适应帮助应对挑战,长期反复的适应过程可能伴随累积成本。

适应为什么有成本?

Allostasis 强调「通过改变来维持稳定」。面对需求时,身体可以调整不同系统的活动;一个反应在短期情境中有用,并不意味着持续维持它也同样有益。[1]

下面的曲线只帮助理解「响应之后如何变化」。它没有真实生理单位,也不用于计算个人的适应负荷。

四种累积路径

经典框架讨论了几种可能伴随累积成本的模式:频繁受到挑战;面对重复挑战仍缺少习惯化;反应在需求结束后持续;以及某些系统反应不足,导致其他系统代偿性活动增强。[3]

因此,重点并不是把所有反应都压得越低越好,而是理解响应是否与情境相适应。

研究如何测量?

研究者常把多个生理系统的指标组合成一个指数。不同研究使用的指标、阈值与计算方式并不一致,测量的可比性仍是持续讨论的问题。[4]

所以,一次皮质醇检测、一个 HRV 数值,或一份疲劳问卷,都不能单独代表这个多系统研究概念。读到一项研究时,需要先看清它实际测量了什么。

已知与未知

我们有一套有用的框架,用来思考长期挑战与多系统调节的联系。但概念本身、特定指标的预测能力,以及具体干预是否有效,是不同的问题。

本文为基础概念导读,依据文末列出的精选文献。阅读时应结合具体研究人群、观察指标和方法,理解结论的适用范围。

科学理解,也包括清楚地知道证据在哪里结束。
