把忙碌的一周,放到更长的时间里看

一个临近的截止日期、几晚被打断的睡眠,还有家里的责任,可能同时出现。它们提出的要求并不相同。科学关心的是:反复出现的要求,以及身体为此作出的响应,怎样与更长时间里的身体变化联系起来。

适应负荷(Allostatic Load)指反复或持续适应可能伴随的累积生理成本。McEwen 与 Stellar 在 1993 年将这一概念引入压力与疾病的研究讨论。[1]

它关注的是一段时间里的调节过程。仅凭「这一周很忙」,我们还不能判断已经发生了多少生物学变化。

生活提出的要求、身体作出的响应,以及反复适应可能留下的成本,是故事中的不同部分。

响应帮助我们应对眼前的要求

动态适应(Allostasis)描述身体通过调整来维持功能稳定。大脑协调自主神经、激素、代谢与免疫等相互作用的过程。同一些介质可以帮助适应,也可能在不同的激活模式下参与不利变化。[2][3]

活动水平发生变化,本来就是这些系统的工作方式。某个介质升高,并不自动等于损伤;解释需要考虑响应的性质、发生情境与持续时间。[3]

下面的示意图追踪一次响应随时间的变化。它没有生理单位,也不计算个人的适应负荷。

响应模式可以怎样发生变化

经典框架区分了几种情况:频繁遇到挑战、面对重复挑战缺少习惯化、响应结束得较慢,以及某一响应不足使其他过程增加代偿活动。[3]

它们分别追问:多久需要启动一次响应?重复之后是否有所适应?什么时候平息?系统之间怎样代偿?这些是组织研究的概念,并不是供人自行对号入座的四类症状。

它与「觉得压力很大」有什么不同

觉得压力很大,描述的是一种体验;压力源描述的是要求或挑战;适应负荷关注的则是适应过程可能产生的累积生理成本。[1][3]

这些描述可以互相提供线索,但不能互相替代。感知压力问卷与多系统生物标志物指数测量的并不是同一件事。一个人的感受有意义,却不能代替尚未测量的生物学指标。

这也帮助我们区分解释与诊断。理解这个框架,可能让一段忙碌生活更容易被说明,但不能据此确诊疾病或认定病因。

一个适应负荷指数里,装了什么

研究者通过组合生物标志物来具体测量这一概念。2022 年的一篇综述纳入了 18 篇综合性综述,考察所用标志物、计算方式,以及与社区研究有关的数据库。不同研究的指标选择和计分方法并不统一。[4]

比较两个分数之前,需要先看涵盖了哪些系统、怎样确定阈值,以及是否考虑用药等相关因素。名称相似,不代表两项研究测量了相同的生物学特征。[4]

一项大型多队列分析探索了更简便的指标组合。某个精简组合在所研究结局中表现有用,并不自动意味着它已成为普遍认可的临床检测组合。[6]

科学概念,比任何一种具体的指标配方都更宽。

与健康结局有关联,能说明什么

一项系统综述与荟萃分析发现,较高适应负荷与死亡结局有关,同时也发现研究之间存在显著差异。这类证据有助于判断指标与后续结局的关系。[5]

干预是否有效,则是另一个问题:改变某种暴露或采用某种措施,能否改变结局?指数与死亡的关联,不能证明某种产品或做法通过降低这个指数就能带来相应获益。

测量文献本身也需要评价。2022 年综述指出,将其所选质量评价工具用于这些文献时存在局限。把一篇论文称为「综述」,并不会自动解决方法上的全部问题。[4]

这个框架为日常理解增加了什么

它邀请我们把视野放到单次事件之外。在忙碌一周的例子里,可以分别看反复出现的要求、要求之间的间隔,以及我们真正想了解的功能。

读到研究结论时,也可以分开三个问题:研究了什么要求或暴露?测量了什么生物学变化?这种变化又与什么结局有关?这样,一句笼统的「压力影响健康」就会变得更具体。

这是一种读懂研究的方式,不是个人负担的计算方法。它也把一个人周围的条件纳入视野:要求、资源与支持,都是适应发生的情境。[3]

证据还在向哪里延伸

适应负荷将一个重要的科学思想,与多样的测量研究连接起来。概念定义、指数构建、预测能力与干预效果,仍然需要分别回答。

本文沿着精选的概念、测量与结局文献展开。接下来可以继续追问恢复:要求结束后,什么发生了变化?追踪的是哪项功能?观察了多久?
