同样的生物标志物,为什么会得到不同分数?

参照人群、调查权重与计分规则如何改变五指标适应负荷指数:一项可复核的 NHANES 方法案例。

测量值不变,分数仍可能改变。

综合指数取决于生成它的规则。在这项 TAP 分析中,仅将参照人群从 40 岁以上改为 18 岁以上,同一批 3,152 人中就有 758 人(24.0%)的总分改变,而他们的生物标志物测量值没有变化。

我们使用 NHANES 2015–2016 数据,比较五指标适应负荷指数的七种计算方案。这是一项描述性方法案例,用于量化分析选择的影响,不判断哪种方法更健康、更准确,或更能预测某项结局。

本案例的方法来源

Beese 等人提出了从二次数据构建适应负荷指数的工作流与代码仓库。我们检查了 2025 年 8 月 1 日的 medRxiv 版本,以及 CHORDS 的固定提交 2bbb624dd01cec5c54b778d3cd2df4ada582c989。预印本与代码分别记录版本。五指标的选择参考了 McCrory 等人的多队列研究。[1][2][4]

以下数值由 TAP 重新计算,不是原论文报告的结果。我们未复现作者的 All of Us 或 HRS 队列,也未运行其完整笔记本。CHORDS 对照方案重建一个 NHANES 周期的合并范围和计分选择,其他比较属于 TAP 的分析选择。

研究了谁?为什么以 40 岁以上为基准?

通过唯一 SEQN 编号连接 CDC 六份公开组件:DEMO_I、GHB_I、HDL_I、HSCRP_I、BPX_I、BMX_I。9,971 条人口学记录中,9,544 人的体检权重为正。符合条件的 40 岁以上成年人共 3,610 人,其中 3,152 人五项齐全,458 人存在缺项。全部分数比较始终使用相同的 3,152 名完整案例。[3]

选择 40 岁以上,是为了使年龄下界与 McCrory 等人所研究的中老年群体大体衔接。这是明确公开的分析基准,不是通用正常范围,也不证明按年龄设置阈值更优。18 岁以上和混龄参照的比较用于展示这一选择的影响。[2]

使用单一调查周期,避免重叠年份重复纳入及跨周期检测方法协调。这些美国历史数据不估计当前健康状况,也不提供中国用户常模。完整案例覆盖符合条件者的 87.3%,按 MEC 权重计算为 90.2%;缺失问题仍然存在。

五指标指数如何计算

五项指标为 CRP(mg/L)、HbA1c(%)、HDL 胆固醇(mg/dL)、调查脉搏(次/分钟)及腰高比(腰围和身高均用 cm)。四项指标在第 75 百分位及以上计 1 分;HDL 在第 25 百分位及以下计 1 分,总分为 0–5。这些阈值来自参照样本,不是临床界值。

基准方案使用 40 岁以上符合条件者中各指标的可用观察值,以 WTMEC2YR 确定阈值。加权分位数采用 Hmisc wtd.quantile 的 type="quantile"、normwt=FALSE,与所检查的上游调用一致。不加权方案使用 R 的 type-7 分位数。这是一种明确的分位数约定;使用权重不会消除所有偏倚。[3][5]

任一指标或阈值缺失时,总分保留缺失。保留 CDC 的检测限以下替代值,其中完整案例有 89 人被标记为 CRP 低于检测限。不进行插补或统计离群值排除。调查脉搏不能直接等同于可穿戴设备的夜间静息心率。

哪些选择改变了分数?

以下全部差异均相对于 40 岁以上基准,参与者及其生物标志物测量值保持不变。「分数改变」指总分至少相差 1 分。表格列出确切人数和未加权比例;第一张图另行展示同一完整案例样本内的 MEC 加权比例。

MEC 加权比例以观测到的完整案例为条件,不估计全国人口发生率或置信区间。

分数改变的 MEC 加权比例:CHORDS 36.4%,全部年龄 36.7%,18 岁以上 23.4%,不加权阈值 12.8%,严格边界 12.7%,完整案例参照 5.4%。

为什么小规则也会产生影响

仅改变等号处理,454 人(14.4%)的总分就改变了。离散或取整的测量会产生并列值:181 人恰好位于基准 HbA1c 阈值,180 人位于脉搏阈值,91 人位于 HDL 阈值;这些人数之间可能重叠。因此,四分位规则不一定恰好标记 25% 的观察值。

CHORDS 的实验室合并以 GHB 为起始表,再左连接其他实验室表;体检合并以 BPX 为起始表。这会限制参照范围。我们单独重建这些连接,并与使用各组件全部可用年龄的方案比较。二者并不相同,尽管总分改变比例四舍五入后均为 39.3%。

独立核对了什么

Python 计算通过第二种 XPT 读取器和独立的逐行实现复核。38 条 CDC 数据字典频数、500 个合成加权分位数测试均一致。识别并纠正了本机 pandas 的零值解码问题,最终分析使用 pyreadstat。

进一步使用原生 R 4.6.1、foreign 0.8.91,以及通过校验的 CRAN Hmisc 5.3-0 原始加权统计函数复核。R 独立读取六份原始 XPT、重建参照范围并计算得分。35 个阈值、22,064 个「参与者×方案」得分以及七组汇总结果均在指定数值容差内一致。

此次在 R 中直接执行 Hmisc 函数及其辅助函数,没有运行其完整软件包或 CHORDS 的完整笔记本。计算一致性说明实现层面的可核对性,不代表构念效度、临床用途或已经完成真人科学审阅。

这项分析尚不能说明什么

MEC 加权汇总以五项齐全者为条件。我们没有估计基于复杂抽样设计的置信区间,也没有针对指标缺失重新校正权重。完整与不完整案例的公开年龄均值分别为 59.3 岁和 62.7 岁,公开年龄在 80 岁封顶。进入完整案例样本的选择过程可能影响解释。

未对药物、疾病或妊娠进行建模处理;未评价临床界值法、连续指标法、死亡预测、纵向变化、诊断准确率或样本外验证。观察到的敏感性不能确定哪种方案更优。

本分析未预注册。本地方案记录了首次运行后的两项实施修订:纠正零值解码,以及增加 CHORDS 特定合并范围的对照。主要评价人群与基准科学规则保持不变。

使用一个分数之前,应该问什么

先问清指标与测量条件、参照人群、权重和并列值处理,以及缺失时如何计算。一个分数应当同时携带这些定义,以及支持其拟议用途的证据。

对 TAP 而言,本案例支持将测量选择公开并做成可复核的流程。生物标志物指数与事件后恢复轨迹如何关联,仍是另一项纵向研究问题。本研究不提供通用 0–100 复原力分数的换算。

这是 TAP 方法案例 v1.0,使用分析规则 v0.1.0。来源检查、编程、计算核对、写作与翻译使用了 AI 辅助。尚未完成具名独立科学与方法审阅;下载材料单独记录已实际执行的计算检查。
